Are you an experienced Alcohol and Other Drug (AOD) clinician looking for a new challenge with Gippsland's largest AOD & Counselling team?
Latrobe Community Health Service Limited (LCHS) is one of the largest community health providers in Gippsland and the Lead agency for Alcohol and Drug Services in Gippsland. In partnership with Gippsland Lakes Community Health, Bass Coast Community Health and Gippsland South Community Health we are working together to drive the new Alcohol and Drug Reform strategy 2012-2015, providing a more streamlined, accessible, high quality evidence based treatment service which is more recovery orientated and family inclusive.
Care and Recovery Coordination seeks to implement and support integrated treatment and care pathways for high risk, complex clients within alcohol and drug treatment services.
The program implements and facilitates a coordinated, strengths based, person centred approach which is individually tailored to the person’s needs. The program supports meaningful involvement by the person and their family in care coordination and goal setting to maximize opportunities for greater social and economic participation. A ‘wrap around’ service model is a key component of the care and recovery program.
The ideal applicant will have an undergraduate degree in a related field and a demonstrated understanding of the principles of the Victorian AOD reform strategies.
For more details or information about this opportunity please contact Jo-Ann Brailey, Manager AOD & Counselling, on 1800 242 696. Applications close 11pm, Sunday 15 July 2018.
