Who We Are

Primary and Community Care Services (PCCS) coordinates multiple services across different sectors to ensure that people's complex unmet needs, including mental and physical health, social, environmental, and inclusion needs, are addressed through collaborative and integrated services that improve their quality of life.

  • improve outcomes for people who are marginalised or disadvantaged, living with significant health or mental health issues, or a disability .

The Role 

  • Enrol patients to the appropriate programs and develop relationships with primary and secondary care organisations to promote the value of our programs
  • Ensure clinical quality and patient centred care is provided to participants of the various program, particularly those that help minimise preventable hospital admissions
  • Complete OT assessments as required fro the team
  • Design and undertake OT therapies
  • Evaluate therapies as required for funder analysis
  • Undertake care planning activities and ensure services to program participants are managed seamlessly and effectively
  • Liaise with various health and social service providers to ensure participants receive timely home and community based care
  • Work closely with GP's and other referrers to support the consumer to access the most appropriate service to meet their needs
  • Ensure timely communication of participant information to the wider members of the care team
  • Accuracy and attention to detail in record keeping and evaluation duties essential

What You'll Need to Succeed

Knowledge

  • Knowledge and understanding of occupational therapy assessments, treatments and evidence based practices appropriate to mental health, long term injured workers, aged care etc 
  • Knowledge of NDIS especially in relation to OT services within the NDIS
  • Knowledge of theoretical models of occupational therapy (i.e., Model of Human Occupation and The Canadian Model of Occupational Performance and Engagement) and how occupational wellbeing can impact on mental health recovery

Skills

  • Occupational Therapist Level 1 or 2 with a recognised degree in Occupational Therapy and current registration with the Australian Health Practitioner Regulation Agency (AHPRA). 
  • Good written and oral communication skills. 
  • Able to formulate care plans and share well rounded clinical reasoning skills.

Experience

  • Experience in psychosocial or other disability, including the capacity to plan, implement and evaluate carer plans
  • Demonstrated group and individual service delivery 
  • Demonstrated ability to work in a multidisciplinary team and organise your work in an effective manner.
