About Us
Prompt Care was established in 2012 to improve support for older people and their families in communities.
We have nurse led teams for our Commonwealth Home Care Package (HCP), Commonwealth Home Support Programme (CHSP), Short Term Restorative Care (STRC) and Veterans Home Care (VHC) clients across Victoria, South Australia and Tasmania.
At Prompt Care, we believe every Australian has the right to live and grow older within their local community. However, many older Australians who are living at home and require nursing or dementia care are left without any alternative but to move into a residential care facility, when their preference is to remain in their own home. The Role
Your role will be to provide Occupational Therapy for our CHSP, HCP, STRC and VHC clients with the key responsibilities including:
- Adhering to Commonwealth reporting standards
- Perform Home based assessments
- Personalised recommendations for our clients
- SWEP Applications and management
- Care Planning
About You
You will be fully registered with AHPRA, have a degree in Occupational Theory and you will ideally have:
- Skills to work in a team environment
- Excellent interpersonal and communication skills
- Ability to work with multicultural clients
- Full drivers license
- Dedication to your profession
