Position Summary

The Screening, Recruitment and Short Term Support Clinician sits within Peninsula Health's Community Health Independence Program. This program is based on the Wagner Chronic Care Model, an evidenced based systems framework. The team both arranges appropriate brokered supports and services for those patients returning home from hospital and also participates in the assertive screening and recruitment of clients eligible for care co-ordination to the Community Care Program.

Key Responsibilities

- Responsible for working with the referring physician, patient/family, acute and sub-acute health care professionals, case management associates, external agencies (when indicated) and other health care providers to promote a smooth, efficient and customer focused transition from acute to Community Care services.
- Ensure screening and recruitment targets people with complex needs that frequently use hospitals or are at imminent risk of hospitalisations and could benefit from co-ordinated care.
- Provide initial needs assessment of clients who consent to participate in the program.
- Effectively coordinate and communicate post acute services to clients, service providers and staff both within Peninsula Health and other health organisations within already established guidelines.
- Provide information and referral to services in accordance with funding guidelines and established service guides.
- Where appropriate, provide short term care coordination support to clients eligible for the program but who potentially only require limited intervention.

Essential Criteria

- Tertirary qualifications related to health service delivery.
- Experience and knowledge regarding chronic disease management.
- Experience with working with at-risk and vulnerable people with complex needs.
- Well-developed written, verbal, interpersonal and computer literacy and communication skills.
- Experience in working both independently and as part of a multi-disciplinary team, with the ability to work effectively, supportively and flexibly.
- Understanding, knowledge and experience implementing service coordination frameworks within a health setting.
- Current Victorian Drivers License
- Demonstrated proficiency in the Microsoft Office suite of programs and other data base software programs.
