The role will call on your advanced clinical and collaborative skills as an experienced Registered Nurse. You will be co-ordinating the multidisciplinary care for clients who frequently access the Orange Health Service Emergency Department with complex physical and psycho-social needs. You will assist clients to increase active involvement in their own health care planning and improve their health outcomes by having more effectively coordinated care. You will bring your broad experience working with a range of clients. You will have a strong interest in working in a new and developing field of practice and be keen to try new ways of providing care to your clients. You will be interested in understanding your client's story and be able to help other service providers to contribute to this holistic perspective of the clients. You will bring a calm and capable approach to your work, and your passion for seeing your clients flourish. We will provide a supportive and engaging project environment. What you'll be doing The Registered Nurse - Integrated Care Coordinator is responsible for coordinating patients' health and social services in partnership with the clinical care team and will support access to services that address their identified needs. The Integrated Care Coordinator will support the Integrated Care Project located within the Orange Health Service. Selection Criteria Current registration with AHRPA and significant experience in the delivery of nursing care and an understanding of the delivery of Primary Health Care.Demonstrated effectiveness engaging clients experiencing complex physical, social and mental health challenges for improved health outcomes.Demonstrated understanding of health coaching or other strategies employed to promote change in health care behaviours.Demonstrated effective written, verbal and interpersonal communication skills.Demonstrated ability to work independently and collaboratively as part of a multi-disciplinary team, providing planning and care co- ordination to meet the needs of clients.Demonstrated information and communication technology skills in standard software programs and electronic patient records for purposes such as word processing, internet, data entry, basic spreadsheet use, email and clinical documentation.Demonstrated skills & ability to organise work priorities and manage time effectively in a high volume work area.Current licence to drive in NSW and willingness to travel to fulfil the responsibilities of the role.
