Palliative Medicine Consultant for the Silver Chain Community Palliative Care Service, Western Sydney.  Part time

Applications are invited from suitably qualified specialists in palliative medicine to work with our dedicated multidisciplinary team in our new community based palliative care service in Western Sydney. The position is available to commence as soon as negotiated, and will be a minimum of 3 days per week.

Silver Chain Group is a not for profit NGO delivering community health care across Australia for over 100 years. They are the largest provider of non-government community based palliative care services across Australia. Silver Chain commenced provision of a 24 hour community palliative care service in Western Sydney in July 2017, based on the successful model used in Perth. This is being provided in partnership with WSLHD. The multidisciplinary community team is led by the Clinical Nurse Consultant Manager.

This is an exciting opportunity for an experienced Palliative Medicine Specialist to provide medical leadership for their team of doctors, and clinical support for the developing service.

Duties include

• Medical leadership for this service.
• Provide community clinical palliative care services, provide collaborative support to General Practitioners, and participate in the community palliative care on-call roster.
• Use your experience and knowledge and become involved in palliative care medical education and capacity building with community clinicians and local GP’s.
• Supervise and train GP and Palliative Medicine Advanced Trainee registrars.
• Work closely with the Supportive and Palliative Care Services in Western Sydney and with other specialties to provide integrated care and seamless care transitions.

Selection Criteria

• FRACP or FAChPM or equivalent Consultant qualification.
• Registered (or registerable) as a Specialist Medical Practitioner in Palliative Care Medicine in Australia (AHPRA)
• Demonstrated current clinical practice of a high standard and an ability to adapt clinical practice in accordance with contemporary evidence based best practice.
• Demonstrated experience in teaching palliative and end of life care to health professionals
• Demonstrated participation in training and supervision of registrars
• Demonstrated sound, effective communication skills
• Demonstrated ability to work in a team
• Willingness to participate in phone on call for the community service 
• Current Driver’s Licence
