An exciting opportunity exists for a Discharge Planner to join our dynamic team at Peninsula Private Hospital.
As Discharge Planner your responsibilities will include:
- Planning, assessing, implementing and evaluating nursing care in collaboration with patients, their families and the multidisciplinary team so as to achieve identified goals and health outcomes.
- Take a leadership role in the coordinatation, supervison, clinical education and direction and support for nursing staff in respect to all aspects of discharge planning.
- Provide support and guidance to nursing peers and patients for the management of their health, recovery, rehabilitation or end of life needs in relation to discharge needs.

Peninsula Private is an acute hospital with a Medical, Surgical, Rehabilitation, Day of Surgery Unit and Operating Theatres.
Selection Criteria:
- Demonstrated effective communication, interpersonal and team building skills
- Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- Commitment to personal and professional development
- Good Interpersonal and time management skills with an ability to work within a multidisciplinary team
- Demonstrated ability to deliver high quality patient care
- Supports and facilitates the patient and family to make informed decisions by providing appropriate information and options regarding health and nursing care
