The Community Aide is an integral member of the Transitional Aged Care Program (TACP).This position is based in Community Health, Prince of Wales, Randwick. Where you'll be working The Community Aide visits clients in their home - located within Randwick - Botany Local Government Areas. What you'll be doing The vision for South Eastern Sydney Local Health District (SESLHD) is ‘exceptional people, healthier lives’. SESLHD is committed to enabling our community to be healthy and well, and to providing the best possible compassionate care when people need it. The Transitional Aged Care Program (TACP) is a slow stream home based therapy program for older people living at home. Working as a valued member of the TACP team, the Community Aide provides a high standard of individualised care to clients under the supervision of nursing, allied health and other clinicians, and is accountable to the TACP Manager. The role involves visiting clients at home, following the client care plans as set by TACP clinicians and providing feedback to clinicians and/or the TACP Manager if any client issues arise. Current unrestricted NSW Driver's Licence Class C Demonstrated knowledge and and understanding of older people and their carers Demonstrated ability to work effectively as part of a multi disciplinary team Experience in attending to client's personal care or willingness to support these activities Demonstrated good written and oral communication Computer literacy and willingness to learn new computer programs. Ability to document in electronic medical records Knowledge of or experience in the safe manual handling of patients or clients Good organisation and time management skills Need more information?
