An exciting opportunity exists for an enthusiastic and experienced Social Worker Case Manager/Practitioner to work in our STAR Health Community Connections Program (CCP). CCP provides coordination of services, information and assistance from a case management framework for people with complex needs who are homeless, at risk of becoming homeless or living in insecure housing
. Clients are met via assertive outreach in their own environments such as parks, supported residential services, rooming houses, low cost private hotels, drop in centres and other places. This process is collaborative, flexible and includes a multidisciplinary approach including secondary consultations with other services. Join our dynamic skilled multidisciplinary team located in St Kilda.
To be suitable for the role you will possess the following:
• A professional qualification in Social Work or similar health discipline.
• Significant experience in case management, service coordination and supporting disadvantaged and marginalised people with complex care needs which may include mental health, drug and alcohol and other health issues.
• Experience in assertive outreach.
• Strong working knowledge of the homelessness and housing sector and ability to access such services.
• Ability to navigate a wide range of housing, health and welfare support services.
• Clear understanding of the issues experienced by people who are homelessness.
• Significant experience in client advocacy
