A great opportunity to work full time in purpose built General Practice in Biloela QLD. This practice is committed to providing individualised Health Care at its highest level. The practice is looking for an experienced RN with current registration to work in this rural practice.

The Candidate:

Your role as a the Chronic Disease Nurse is to coordinate the chronic disease programs and provide an exceptional standard of care to our patients. Working collaboratively with the General Practitioners, you will provide assessment, treatment, education and care to all patients. 

RESPONSIBILITES INCLUDE BUT NOT LIMITED TO:

Conduct screening of the patient data base for patients with or at risk of chronic disease, co-ordinate patient Recall and GP management plans and team care arrangement. Assist with patient education and community health promotion activities.  

Support the other nurses when required with clinical duties within required level of clinical competency, according to best available evidence, providing patients with support and reassurance.

Assist with triage, data management, diagnostic services, networking with other providers, planning & management of patient care and patient advocacy.

Ensure sterilisation and maintenance of clinical equipment and consumable stock maintenance and ensuring vaccine stock potency.

Maintain awareness of current and new legislation to ensure business is complying with all statutory and regulatory obligations including infection control, sterilisation, hazardous materials & safe handling/disposal of medical waste, records management, OHS, and accreditation.

KNOWLEDGE AND TECHNICAL EXPERTISE

ESSENTIAL:

Appropriate tertiary qualification & registration certificate.

Minimum of three (3) years nursing experience, preferably in a community/rural based role.

Previous training in clinical supervision

Demonstrated understanding of the needs of general practice and interest in community/rural based nursing including patient education and health promotion. Knowledge of current, Diabetes management, asthma management, enhanced primary care items, immunisation schedules, infection control, CPR and emergency resuscitation techniques, equipment & drugs.

DESIRABLE:

Competence in the use of relevant equipment & software (Best Practice)

Experience in similar role as practice nurse or community/rural based nursing.

Experience with specific social groups serviced by practice, such as children, postnatal women, breastfeeding women.
