Community / HomeCare Registered Nurse: South West Sydney: Aged Care

The Role 
This role reports to the Aged Care Operations Manager. You will work together to ensure clients needs are being met. You will coordinate the development and delivery for services to their clients (mostly level 3) within the local area. You will be overseeing the Case Management of clients and overseeing a team of carers. 

This role is a fantastic role offering a mixture of hands on nursing, leadership and case management. Liaise with Sydney West community clients, running assessments, developing care plans, admitting new clients to community care programmes, including home care packages.

Requirements:
This very busy and highly varied role that offers a real challenge! We are looking for someone with superior communication and people skills. You will enjoy working with the elderly, people with a disability and be a team player with:

• Bachelor of Nursing 
• Experience in Home Care/ Community services (aged care, disability).
• Case Management experience 
• Previous experience leading and managing staff
• A current Australian Drivers Licence
• Superior interpersonal and communication skills
• Excellent communication, time management and computer skills
• Strong attention to detail and the ability to effectively problem solve
