An exciting opportunity is now available for a NDIS Aboriginal Support Worker to work on a full-time basis, at our Guildford site.
Role Overview
The 360 NDIS Support Coordinator (SC) coordinates the care of NDIS participants, liaising with all relevant stakeholders.
A SC is appointed by the participant who has an NDIS plan. The SC is responsible for working with the participant to connect with support services in line with their NDIS plan. These connections will enhance opportunities to pursue existing goals which are appropriate to their age, gender, culture and religion and build the person’s capacity to achieve greater independence to self-direct their own services in the longer term.
Essential Minimum Selection Criteria
Experience/Qualifications
- Allied Health Qualification in either Psychology, Social Work or Occupational Therapy or equivalent experience;
- Min 3 years experience within and an understanding of clinical/health and / or welfare services and support systems;
Skills, Abilities and Knowledge
- High level interpersonal skills, including the ability to communicate sensitively and effectively with Aboriginal and Torres Strait Islander people;
- Knowledge and understanding of local Aboriginal and Torres Strait Islander communities and cultures;
- Respect within local Aboriginal communities as a responsible community member;
- An interest in Aboriginal health issues and enthusiasm to provide assistance and care to others in the community;
- An understanding of mental health issues and / or experience working with people with severe mental illnesses and complex needs and their families;
- Strong communication, written and negotiation skills;
- Experience in analysing and formulating client assessment and plans;
- Demonstrated ability to work unsupervised as well as within a team environment;
- Experience in building and maintaining effective relationships and partnerships with other Clinical and Community Support Organisations;
- Ability to work with a variety of stakeholders including clients, carers, GPs and Clinical and Community Support Organisations;
