About the Role
- Provide patient centred care coordination across the patient journey by enhancing system integration and reducing fragmentation and barriers to care.
- Provide a central point of communication, encouragement and coordination across primary, secondary and tertiary care providers utilising multimodal communication pathways with a patient centred care pathway, ensuring that the patient is the centre and most important collaborator in the care journey with all stakeholders.
- Foster and encourage active patient engagement in the development of health care goals to promote self- management and seek to improve patient health literacy.
- Enhance health literacy to support patients and families to make informed decisions about their health care options including advance care planning.
- Apply advanced specialist knowledge, experience and clinical skills of contemporary nursing practice and theory and participate directly in the support of patient centred care.
Salary Information:
The total remuneration value of up to $133,535 p.a., is comprised of: salary between $107,804 - $117,037 p.a., (Nurse Grade 7); employer contribution to superannuation (up to 12.75%); annual leave loading (17.5%); Permanent Full Time (two positions available) Applications will remain current for 12 months.
