Carer Peer Support Worker – Post Discharge Support

  • Part-time, 40hrs/fortnight

Life with us

Broadmeadows Inpatient Psychiatric Unit is a 25 bed unit including a brand new 5 bed ICA area. We work within the recovery model and have a multidisciplinary approach to care of individuals diagnosed with, or at risk of developing a serious mental illness. We will welcome you as part of a caring, multi-disciplinary team to work with people with challenging and complex mental health presentations.  You will have the prospect to make a difference to our consumers and families with whom you work with.

  • Sharing skills and learnings as appropriate, in a way that brings hope, supports and empowers families / carers to move forward in their lives;
  • Assisting in the identification of consumers, and their family / carer / support people, admitted to the acute mental health inpatient unit who are at risk of readmission after discharge;
  • Contributing to effective discharge planning including establishing peer worker / carer / family contacts both during and after the inpatient stay;
  • Collaborating with clinicians in referring carers to community programs, education and support networks, as appropriate;
  • Supporting carers to be involved in decision-making in relation to their consumer’s care and treatment;
  • Assisting carers to identify their support needs and focus on their own well-being

Your contribution

  • Working as part of a dynamic group of peer support workers (consumer and carers) as an integral part of the multi-disciplinary team;
  • Providing individual and group peer support using the Intentional Peer Support model;
  • Experience providing support for a family member (partner, parent, grandparent, child etc.) with a mental illness treated within the adult mental health system;
  • Ability to work from a recovery and strengths based framework where people are the experts in their own lives;
  • Ability to communicate effectively with families / carers, consumers, colleagues and other service providers through demonstrated interpersonal skills.
