Alternate Care Project Coordinator

About the Organisation
Central Queensland, Wide Bay, Sunshine Coast PHN (The PHN) is an independent not-for-profit organisation funded by the Commonwealth Department of Health to improve the health outcomes of the region. It covers over 169,500 km2.
The PHN coordinates primary and preventative healthcare – that is, the healthcare that takes place outside of a hospital, such as General Practitioners (GPs), allied health, chronic disease management, aged care, mental health and Indigenous health.
The key objectives of the PHN are to:
- Enable responsive and dynamic solutions that optimise population health and wellbeing
- Build healthy, resilient, connected communities – country to coast.
About the Opportunity
The Alternate Care Project Coordinator will:
- work to improve integration, including information sharing & case coordination between Child Safety, families, carer and primary health care teams for children and young people in alternate care.
- work with the primary health care sector to develop and embed knowledge and skills for working with children & young people in alternate care; and
- improve access, timelines & quality of health assessments for children in alternate care
Key selection criteria:
- A relevant tertiary and/or experience in health management and planning, population health, social services, project management and/or a program specific discipline
- Demonstrated knowledge & experience of working within health projects as well as child safety/human services
- Experience in effective stakeholder engagement, relationship development and solutions design with community service providers, consumers, carers, advocates, providers and others
- Experience in engaging with Aboriginal and Torres Strait Islander communities, services & consumers and designing solutions that reflect their needs.
