What you will be doing

The Child and Family Health Nursing service forms an integral part of primary health care. It aims to maintain, improve and promote the optimal health and wellbeing of children aged 0-5 years and their families by providing safe, compassionate care through the assessment, planning, implementation and evaluation of nursing care in collaboration with families and the multidisciplinary health care team to achieve safe quality care outcomes that meet the goals of the child and family. It acts as an entry point into the community health care system and occurs in multiple settings within the community, supporting an integrated multidisciplinary service response based on individual client and family needs.

The Child and Family Health Nurse practices within a primary health care framework to provide a comprehensive range of child and family health services to infants, children, their parents, carers and families in pregnancy, where required, through to five (5) years of age. The nurse utilises the Family Partnership Model as the foundation for the nurse-family relationship and supportive counselling to deliver programs such as Universal Health Home Visiting and Safe Start.

The nurse will provide a screening and surveillance program for infants and children as informed by the NSW Personal Health Record using evidence-based tools and provide opportunistic immunisations according to the NSW Childhood Immunisation Schedule (0-5 years). The nurse will actively promote, protect and support breastfeeding and facilitate evidence-based groups delivered to meet child health, development and parenting support needs of families within the community.
