An opportunity currently exists for a Grade 2 Occupational Therapist to join the St Vincent’s Home Based (Domiciliary Therapy) Allied Health team, located at St George’s Hospital, Kew

Department
The St Vincent’s Home Based (Domiciliary Therapy) Allied Health team is jointly funded by the Victorian Home and Community Care Program for Younger People (HACC-PYP) and the Commonwealth Home Support Program (CHSP), and focuses on restoring, or maintaining people’s independence, improving functioning and wellbeing to support them to remain living at home. The team includes: Physiotherapy, Occupational Therapy, Speech Pathology, Podiatry, Dietetics and an Allied Health Assistant.
Responsibilities of the role
Your responsibilities will include:
- Assess functional status of clients in their home, identify dysfunctional component and recommendations for appropriate adaptation.
- Undertake home visits to identify structural and environmental barriers.
- Assess for, and arrange supply of equipment to clients to improve safety, maximise functional ability, mobility and independence
- Use a problem solving approach involving assessment, planning, implementation and evaluation in the provision of quality care to the client/family.
- Liaise closely with community service providers and attend relevant meetings to coordinate client care
- Delegate activities to, and mentor the Allied Health Assistant to ensure high level of client care
Key Selection Criteria
To be successful for this role you will require:
- Degree in Occupational Therapy
- Current Victorian AHPRA Occupational therapy Registration (and Eligibility to Australian Occupational therapy Association)
- Current Victorian driver’s license
- Minimum of four (4) years post-graduate experience in a relevant field of rehabilitation and or community based service
