ABOUT THE ROLE:
An exciting opportunity exists for an ACAT Team Leader/Assessor who will provide leadership and direction for the Aged Care Assessment Team, and provide comprehensive assessments of frail older people to facilitate access to services appropriate to their care needs.
- Fixed Term Contract 76 hrs per fortnight until 30 June 2020
- Lead, direct and co-ordinate the multi-disciplinary team, at the Department level
- Ensure the delivery of care is safe and efficient, while maintaining standards of care
- Use your time management and leadership skills to monitor and meet KPI’s
- Undertake the business/management processes of the ACAT Department
ABOUT YOU:
- Current Registration with the Australian Health Practitioners Regulation Agency (AHPRA) as applicable
- Minimum 3 years post grad experience with Aged Care as a Registered Nurse, Social Worker, Occupational Therapist or Physiotherapist
- Demonstrated high level of interpersonal, written and verbal communication skills including computer literacy.
- Current unrestricted Drivers Licence and ability to drive distances for work purposes
- Current Immunisation of diseases as listed in the current NSW Health Policy Directive. This is a category A Risk Position
