Job Description

Banyule Community Health is seeking to employ an enthusiastic and highly skilled Aboriginal Health Case Manager with experience in leading a multidisciplinary team. The Case Manager Clinical Lead incorporates direct clinical care as well as clinical and day to day operational leadership. The position works closely with clients, BCH staff, Aboriginal Controlled Organisations and other service providers to improve the health outcomes for our Aboriginal and Torres Strait Islander community. This role will also provide clinical and day to day operational leadership to the Banyule Aboriginal Health Team made up of Care Coordinators (chronic and complex care needs), Outreach Workers, Early Years Support Worker and Counsellor.  Aboriginal Health Workers strongly encouraged to apply.

Company Profile:

Banyule Community Health has two main sites, one in the heart of West Heidelberg and the other near the train station of Greensborough. The Community Programs Team provides a range of services, including Aboriginal health, prevention and health promotion projects and chronic disease related programs.

The key responsibilities for this position include:

  • To carry a client case load providing case management support alongside other the services provided to Aboriginal and Torres Strait clients, their family, and or carers (counselling, care coordination and early years support)
  • To work alongside Banyule Community Health Aboriginal Health staff and Aboriginal Community Controlled Organisations to deliver culturally safe and appropriate services and support clients to overcome barriers to accessing appropriate health services
  • Provide support and guidance to the Aboriginal Health Team staff as required providing day-to-day support, debriefing and task focused supervision

The key skills required for this role include:

  • Deep understanding of Aboriginal and Torres Strait Islander communities and their experiences of health systems
  • Developed skills in planning, navigating, case management and advocating for clients who are managing complex and chronic health and social circumstances                        
  • Demonstrated ability to mentor staff, support clinical decision making in relation to a range of roles and provide daya to day support, debriefing and task focused supervision
  • Tertiary qualifications in Social Work, Health Science, Public Health or  other relevant field or substantial equivalent work experience
  •  At least five years experience working with Aboriginal people, families and their communities
  • Aboringal and Torres Strait Islander people encouraged to apply
