What you'll be doing The Integrated Care Service (ICS) supports the existing systems to provide and integrated model of care for the patient journey from custody to the community. This position will be responsible to ensure continuity of care for all patients with acute/chronic disease and/or complex health needs from reception into custody to release to the community and thereafter and will be responsible for providing a support network to JH&FMHN clinicians for patients with acute/chronic and/or complex health needs, assisting the local health staff with the care coordination and case management of these patients on an on-going basis. Tertiary qualification in health related field or combination of study and work experience in a health related field Demonstrated understanding of the provision of health care for people with chronic disease conditions and/or complex health needs, with relevant experience such as working in chronic care services and/or community services Demonstrated experience in coordinating/case managing the care of patients with chronic disease and/or complex health needs Demonstrated competent high level communication and interpersonal skills, report-writing, with an ability to engage with various members of the multidisciplinary team and a wide range of people from a variety of backgrounds Demonstrated creativity and innovation in approach to complex problem solving with the ability to negotiate, network, multi-task and manage competing deadlines Demonstrated proficiency in using computer systems in a Windows environment, including MS Office applications and common database/statistics software Understanding of the Ministry of Health Integrated Care Strategy and how it relates to the JH&FMHN environment.
