Our skilled and diverse team provide a broad range of services including acute "Hospital in the Home", "Rehabilitation in the Home", Aged Care, Chronic Disease Management, Mental Health Support, Allied Health, Personal Care, Case Management and Care Coordination.
Opportunity:
To contribute to the care and quality of life for our clients. Working independently you will provide a diverse range of clinical care to patients living in their own homes. Care provided includes; clinical assessments, pain management, exercise prescription, equipment provision and falls prevention.
Skills & Experience:
- Minimum 3 years Physiotherapy experience
- Current AHPRA Registration
- Current National Police Clearance valid for Aged Care and Vulnerable People
- Current Drivers' Licence
- Access to a computer, internet and printer (or be willing to purchase)
- Outstanding communication and people skills
- Ability to problem solve and use initiative
- Work both autonomously and within a team
