Our exciting new Health Management program and clinics provide targeted interventions for individuals with complex and chronic diseases, optimising health outcomes and reducing avoidable hospitalisations. Working in our friendly Campbelltown Medical Centre, the Health Management Nurse will work with the practice team and provide coordinated care to enrolled patients. Key responsibilities of the role include:

• Identify and recruit eligible patients to participate in Health Management clinics
• Support patients to better self-manage their conditions through preparation of GP management plans and coordination of Team Care Arrangements
• Conduct health assessments using current and evidence based assessment tools
• Liaise with all members of the health care team to ensure services are well coordinated
• Support GPs, practice nurse and practice staff in program protocols and procedures
• Although this role is predominantly based in the practice, there is a requirement that the successful applicant is confident in providing in-home service as well

Do you have the qualities that we are looking for?

• Current AHPRA Nurse registration - Registered Nurse (Division 1) or Enrolled Nurse (Division 2)
• At least 1 years' post registration experience
• Experience in chronic disease management is desirable but not essential (particularly diabetes experience)
• A valid Working with Children Check or willingness to obtain
• Previous experience working in general practice would be highly regarded
• Excellent oral & written communication skills
• A caring and collaborative approach to patient care
• Proven multi-tasking, problem solving and time management skills
