Job Summary

The Intake Officer receives referrals, screens for eligibility, allocates to providers, or provides alternative options. 

About the business and the role

South Eastern Melbourne PHN (SEMPHN) has the key objectives of:

  • Increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and
  • Improving coordination of care to ensure patients receive the right care in the right place at the right time.

The Intake Officer works as part of the SEMPHN Stepped Care team, and are the central point of entry into the SEMPHN funded services.  The team also facilitates referrals for existing clients that need to step up or down into different parts of the stepped care model according to their needs. As this is a newly established service, the role will also actively participate in activities that inform intake processes, resources and work flows.

Job tasks and responsibilities

In this role you will:

  • Receive and process referrals, screen eligibility and allocate to appropriate organisation for assessment and delivery of treatment services.
  • Enter complete and accurate client information addressing Minimum Data Set requirements into the client management system, Episoft and compliance with the required data collection and privacy standards.
  • Actively assist ineligible clients to access more appropriate services as needed in conjunction with their referring GP or practitioner.
  • Effectively liaise with GPs, other health professionals, agencies and organisations involved in the program as required.
  • Escalate clinical issues/concerns to the Stepped Care Model Intake lead where additional assistance/guidance is indicated.
  • Assist GPs, Allied Health Providers and the general public with information on the Stepped Care Model programs.
  • Adhere to statutory acts, standards and codes of ethics relevant to professional background.
  • Participate and contribute to the development and refinements of intake processes, resources & work flows.
  • Participate in the intake workflows in answering phone calls and enquiries, processing and screening referrals, prioritising and following up tasks in a timely and effective manner.
  • Participate in and contribute to demand management activities.

Skills and experience

The successful candidates will have:

  • Certificate IV in field such as psychology, social work, mental health, alcohol and other drugs or Aboriginal and Torres Strait Islander Health.
  • Experience in intake, assessment and care planning.
  • Experience working with clients with mild, moderate and severe and persistent mental health issues.
  • Experience working with clients with alcohol and other drug issues.
  • Experience working with GPs, Allied Health Providers and clients.
  • Demonstrated experience with understanding the mental health and alcohol and other drugs service system.
  • Demonstrated ability to be flexible and adaptive in a new service environment.
  • Excellent telephone manner and general communication (verbal and written) and negotiation skills.
  • Excellent clinical documentation skills and attention to detail.
  • Ability to work independently and as part of a team.
  • Excellent organisation and time management skills.
  • Ability to troubleshoot and access resources to support accurate decision making outcomes
  • Strong analytical problem solving skills with the ability to analyse issues and provide advice on available options upon which decisions can be based.
  • Extensive experience in the use of Client Management Systems. 
  • Proficiency in the use of Microsoft Office suite.
  • Appointees must have flexibility with starting and finishing times and potentially may be rostered to work weekends.
