The Role:

This role requires a dynamic and experienced Care Manager to support and coordinate a team of aged care workers to deliver a range of 'in-home' care services to their clients.

What you will have:

Care Manager experience in community nursing / home care
Good knowledge of the Home Care packages, TACP & CHSP
A current drivers license and clean, safe driving record
Strong organisational and documentation skills
Good administration and computer skills (including using a Smart phone)
Be able to manager care workers
