Narre Warren Medical Centre, is a busy, fully accredited family general practice.
We are seeking a motivated, enthusiastic and very friendly practice nurse.
The Registered Nurse Div 1 or Div 2 RN with full medication endorsement will be responsible for assisting in the development of nurse-initiated chronic disease management in the practice.
We have a well established Nurse run clinic to ensure best outcomes for all our patents, we are increasing to ensure we keep up with the demand.
Experience in Primary Health Nursing is essential.
Nursing responsibilities include patient liaison, setting up and identifying management of the recall system, patient results, care planning, health assessments, stock control, cold chain management and provision of support to practice staff and doctors, Doppler's, spirometers, ECGs.Cycles of care for all our patients with Diabetes and respiratory disorders.
For 3-4 half days per week the successful applicant will be undertaking Chronic Disease Management Care plans and Health Assessments alternating from primary health nursing , clinical in treatment room then CDM room.
The practice is fully computerized so keyboard and computer skills are essential.
We are fully accredited with AGPAL.
The successful candidate will have a minimum of 3 years experience as an RN 1/RN 2 Endorsed and will possess strong clinical skills and academic skills associated with looking at the patient holistically .
The ability to work autonomously and as part of the team is essential.
We are a quality and very busy practice which strives to maintain the highest level of patient care.
Hours: Negotiable
Expectation will be that the RN can fill in the other 10 weeks of annual leave from nursing colleagues.
As this is a job share position, the ability to assist covering annual and sick leave will be compulsory. Of course variations to this are always considered.
Starting date ASAP. On-the-job training will be provided.
