About the facility
Barwon Health’s McKellar Centre is a recently upgraded facility located only minutes from the centre of Geelong. The centre is a 100 bed inpatient facility offering rehabilitation in Palliative care, Geriatric evaluation and management, Stroke, Trauma, orthopaedic and amputee service. The McKellar Centre offers a range of professional aged care services. It is situated on a vibrant community asset with parkland surrounds and incorporates state of the art equipment.
About the role
This role is 0.2 EFT (8 hours/week) that needs to be a Friday. In this role you will work with 16-25 year olds with complex disability and/or chronic health conditions as they prepare for, and complete the transition from, paediatric health/disability services into adult health/disability services.
Comprehensive team assessments occur followed by shared care planning and transition focused interventions.
Knowledge of local/regional services is required.
