Clinical Care Coordinator
Ambulatory Aged Care Programs
Permanent Part time position 72 hours per fortnight
An opportunity exists for a suitably qualified Registered Nurse or Allied health professional to join the Ambulatory Aged care Programs. This position is a permanent part time position. The role provides case management / care coordination to clients in a community setting on Transition Care, Restorative Care and GEM in the Home programs. These 3 programs form the Ambulatory aged care Programs.
The Clinical Care Coordinator is an integral part of the multidisciplinary team. They will work in consultation with Physiotherapists, OT’s, Team Assistants, Speech pathologists, Dietitians, Geriatricians and Team leaders to implement best practice for AACP clients.
This position will provide the successful applicant with valuable Case Management/Care Coordination skills which is a sought after quality in community health programs. The successful applicant will have high level interpersonal and communication skills and a demonstrated capacity to work successfully in a team environment. Must have an advanced level of clinical skills and be able to demonstrate excellent time management. Drivers licence essential.
