Mentis Assist is a not-for-profit, non-government, community managed Mental Health Service and NDIS provider, supporting people with severe and enduring mental illness and psychiatric disabilities to recognise their values, strengths and goals.
The Role
This role has four core components:
- Assist our consumers with complex and multiple needs to access a wide range of services to improve their mental health and general wellbeing
- Facilitate collaboration between services, the consumer and their support networks to build capacity and provide a wrap-around service
- Chair Care Co-ordination meetings to develop strong care team participation
- Prepare and plan for NDIS transition and implementation with our consumers
Role Responsibilities:
- Navigate the service system and access appropriate supports
- Work closely with clinical AOD, Housing, legal, DV, GP’s, public and private health providers to name a few
- Using a person centred approach, advocate on behalf of your consumers and negotiate with service providers
- In collaboration with the consumer, develop realistic goals and action plans
- Ensure that when the client is ready for discharge, they are able to self- manage and/or be linked with an appropriate care team
Our successful candidate will:
- Have qualifications in Social Work or related allied health field
- Have significant experience in mental health/community services/health sector
- Possess a good understanding of client centered, recovery-focused, family-sensitive case work and care coordination
- Have highly developed organizational skills, excellent communication and interpersonal skills and the confidence to approach health professionals
