The Role
Nurse practitioner needed to work as part of the Nurse Navigation program in QLD. The role is based in a remote region of QLD. The role is for a January start for either a permanent position or 12-month contract. Your role will be to offer care coordination and complex case management to clients with chronic disease within an Aboriginal community. To be successful you will need to have a background in primary health, chronic disease, and emergency care, previous remote experience will also be highly desirable. This is a great role for someone seeking a challenging new position, who enjoying working autonomously while experiencing remote back living.
  
Criteria
In order to be successful, you will need to meet the following criteria:

• Credentialed Nurse Practitioner
• Chronic disease and primary health background
• Previous experience within isolated practice – highly desirable
• Current serology reports
