Church Street Medical Centre is seeking to employ an experienced Div 1 Registered Nurse to join our supportive and successful team in a Quality Improvement and Chronic Disease Management role.

We are a busy, growing General Practice located in central Richmond. We are non corporate, computerised, accredited, with modern, fully equipped rooms; proudly servicing the needs of our local and not so local families for the last 10 years.

We are seeking an experienced Div 1 Registered Nurse, previous Practice Nurse experience preferred.

The successful applicant will be integral in driving our new Quality Improvement and Chronic Disease Management Program. Previous experience as a Practice Nurse, and in QI including data collection, setting up and managing disease registers and chronic disease management including care planning will be highly regarded. The position is Part Time, currently 3 days, with room to increase hours as the role develops. A half day per week will be reserved for Practice Nurse clinical duties. Days worked are negotiable. Above award wages negotiated dependent on skills and experience.

Key responsibilities:

Co-ordinate QI program

Data collection and reporting for QI

Management of disease registers

Co-ordinate chronic care management program

Prepare care plans and health assessments

Chronic disease management

Immunisations - adult & child

Wound management

Infection control and sterilising procedures

Maintaining cold chain management systems

Assisting with procedures

Essential Criteria:

Current Nurse (AHPRA) registration

Previous General Practice experience
Current CPR certificate
Effective patient triage
Excellent interpersonal and communication skills
Excellent computer skills including Microsoft Office suite of programs
Strong time management
Ability to work within a team and autonomously

Australian Resident for work purposes

Desirable Criteria:

Medical software knowledge - Zedmed preferred
Flexibility to cover staff leave
