What you'll be doing Home Support Assessors provide basic home-based face-to-face assessments of clients referred via the My Aged Care Gateway using the National Screening and Assessment Form (NSAF). Working with each client, the Home Support Assessor establishes goals specific to the client; develops appropriate strategies and actions, including referral to age care and non-aged care services; and develops a Support Plan. The Home Support Assessor completes the NSAF and matches and refers clients to appropriate service(s). Certificate III in Home Care or Aged Care or equivalent work experience. Well-developed written, verbal and interpersonal communication skills. Demonstrated computer skills with capacity to learn and use electronic reporting systems. Demonstrated ability to work independently as well as effectively in a collaborative team environment. Ability to follow guidelines relevant to position as well as demonstrated problem solving capacity. Current valid Australian Driver’s Licence (P2 acceptable). Demonstrated understanding of how the District is Caring for the Coast and how this role would contribute. Need more information?
