The Role:

An exciting opportunity currently exists for a motivated, enthusiastic Grade 2 Physiotherapist in the Post Acute Care Service to provide quality physiotherapy to patients and their families post discharge, in their own home environment.

The Post-Acute Care Program at RCH is a stated-wide service coordinated by the Complex Care Hub with care and support provided by RCH staff members and/or external organisations. Post-Acute Care provides short-term support at home to children whose health care needs have increased following an acute hospital admission or Emergency department presentation. The program aims to help children recover at home and prevent readmission to hospital. Services offered as a part of this program include; nursing care, occupational therapy, physiotherapy, personal care and respite.

The Post-Acute Care Physiotherapist will work in partnership with medical, nursing and allied health staff and the Post-Acute Care team. 

The successful applicant will have demonstrated experience working in a paediatric, community or relevant acute adult physiotherapy service. They will also have proven experience with working and communicating with stakeholders at various levels within hospital and community settings.
