About the Role

We are seeking a dynamic and enthusiastic Grade 2 Outpatients / Hospital In The Home Pharmacist to join our team at Footscray Hospital from 3rd September 2018 on a full-time ongoing basis.

This position is as follows:

• Full-time position 80 hours per fortnight from 3rd September 2018 on an ongoing basis.
• 8.15am to 5pm Monday to Friday with potential for on-call and weekend shifts.

This is an excellent opportunity for a pharmacist looking to expand and develop their operational and clinical skills within a hospital environment.  Whilst the position will be based at Footscray Hospital, applicants must be flexible and willing to work across the various campuses of Western Health as required.

Duties

• Provide clinical and dispensing services to the patients and clients of Western Health
• Being the liaison person for HITH and Outpatients and be responsible for the provision and coordination of dispensing services provided from Footscray Hospital.
• Responsibility for updating procedures and processes related to HITH and Outpatients pharmacy services
• Ensuring services are provided in accordance with all relevant legislative, PBS and practice standard requirements.
• Assist in the development, implementation and maintenance of innovative pharmacy services
• Provide expert professional knowledge and skills to ensure that patient outcomes are optimised
• Participate in quality assurance initiatives and practice research projects under the guidance of the senior management team and act as the Dispensary Manager when required.
• Ability to be flexible, assign priority to responsibilities and meet time constraints
• Act as a positive role model to junior members of staff, as well as demonstrate broad experience in providing high quality dispensing services.
• Exhibit excellent communication and interpersonal skills, information technology skills and participate in various multidisciplinary working groups as required.
• Facilitate patient discharge & discharge medication workflow
• Provide expert knowledge within a multidisciplinary team
• Support prescribers to maximize the clinical and financial benefits of medications usage.
• Contribute to education, training and quality activities

Essential Criteria

• Current pharmacist registration with AHPRA
• Permanent resident or citizen of Australia
• Possess excellent clinical & organisational skills
• Ability to be proactive and work responsibly independently
• Demonstrate exceptional interpersonal & communication skills
• Good information technology skills
