WA Primary Health Alliance (WAPHA) is the peak planning and commissioning body for primary health care services in Western Australia (WA).

We oversee the strategic commissioning functions WA's three Primary Health Networks: Perth North, Perth South and Country WA.

WAPHA's primary objective is to improve health outcomes and patient experiences through the commissioning of appropriate services where they are most needed.

The role

Funding has been provided to Primary Health Networks (PHNs) to establish 3 new positions to work with Partners in Recovery Lead Agencies and consortia members, and with Personal Helpers and Mentors (PHaMS), and Day to Day Living Program lead agencies.  People with a persistent mental illness registered with these programs, and not eligible for the National Disability Insurance Scheme are to receive continuity of support packages commensurate with their needs for psychosocial support. PHNs have received short term funds to assume fund holding and commissioning responsibilities for the above programs.

The role will assist lead agencies to transition eligible clients to the National Disability Insurance Scheme and to transition ineligible clients to either the Continuity of Support arrangements or to the National Psychosocial Support measure as appropriate.

Key outcomes expected from the project are:

  • Appropriate levels of psychosocial support are in place for people who meet access criteria and are not eligible for the NDIS.
  • Transition of eligible clients and funding from the Partners in Recovery Program to the NDIS.

Responsibilities and duties include:

  • Work with existing PIR, PHaMS and Day to Day Living Lead Agencies as the contract relationship managers to prepare and monitor sub-contracting arrangement;
  • Establish processes to monitor and assess progress reports and milestones attached to payments;
  • Become a member of the PIR Consortia;
  • Support Lead Agencies and consortia members to transition program members to the NDIS;
  • Support Lead Agencies and consortia members to identify people who are not eligible for the NDIS and who require Continuity of Support.
  • Support the establishment of data collection and storage;
  • Establish a relationship with the local National Disability Insurance Agency office in the PIR region.
  • Identify, report and manage project risks.

About you

The successful candidate will meet the following selection criteria:

  • Relevant tertiary qualifications and demonstrated knowledge and experience in a mental health field.
  • Demonstrated skills in contract management.
  • Experience in health service delivery, evaluation and/or planning.
  • Highly developed interpersonal, facilitation and relationship management skills including the ability to influence and negotiate outcomes.
  • Excellent written skills with ability to communicate complex issues clearly including the preparation of reports and submissions.
  • Highly developed computer skills including proficiency in MS Office the ability to learn new computer applications.
  • Demonstrated ability to work autonomously and as a team member.
  • Previous experience within a Partners in Recovery Program will be highly regarded.

This role is on a 12 month contract.
