Integrated Care Co-design Manager Employment Type: Temporary Full Time Position Classification: Health Mgr Lvl 4 Remuneration: 121246 - 145063 Hours Per Week: 38 Temporary Contract Until:31/01/2020 Requisition ID: REQ54307 The vision for South Eastern Sydney Local Health District (SESLHD) is ‘exceptional people, healthier lives’. SESLHD is committed to enabling our community to be healthy and well, and to providing the best possible compassionate care when people need it. This position will provide subject matter expertise and build collaborative relationships in leading and managing the design, implementation and evaluation of co-design programs of work as enablers for South Eastern Sydney Collaborative Integrated Care Strategy. This program of work will be underpinned by the principles and practices of co-design methodology, improvement science and implementation science. The position will engage, collaborate and partner with the Improvement and Innovation Hub (iiHub), the Primary, Integrated and Community Health (PICH) Directorate, the Central and Eastern Sydney Primary Health Network (CESPHN), the community and other key stakeholders and agencies internal and external to the organisation whose commitment is integral to delivering on the South Eastern Sydney Collaborative Integrated Care Strategy Action Plan. Tertiary qualifications in Health Service Management or in a health-related discipline including public/population health, demonstrated equivalent, relevant professional experience and training or a combination of study and work experience. Proven leadership and management skills and demonstrated experience in forging, empowering and maintaining partnerships with multiple stakeholders. Extensive experience in all aspects of project management. Excellent understanding of contemporary issues and advances in integrated health service design, delivery and evaluation. Demonstrated experience in applying the principles and practices of co-design and improvement science in program design, implementation and evaluation. Demonstrated high level communication skills, with proven experience in facilitation and presentation Demonstrated strategic planning skills and experience, including the ability to make complex judgements and work innovatively in delivering high quality products and outcomes. Current NSW Driver’s Licence and a willingness to travel in accordance with the demands of the position. Need more information?
