Opportunity:
Due to further Company growth we have an exciting opportunity for you to apply your clinical skills to an office based position coordinating in home patient care visits, across a wide range of public and private funded programs.
Duties and Responsibilities
- Use your clinical skills to co-ordinate all patient care needs.
Coordinate a wide range of services for our patients within a multidisciplinary environment - Liaising with patients, hospitals and our team of health professionals on the road.
- Develop seamless transition of care for patients discharging from hospital or referred through our community programs.
- Case manage all care delivery and patient outcomes
Skills and Experience
- Have recent acute clinical skills
- Minimum 2 years experience post grad experience (desirable)
- Computer savvy
- Exceptional communication and organisation skills
- Able to prioritise workload
- Work both autonomously and within a team
- Current National Police Clearance (or be willing to obtain one)
