About the Role: The EACH Community Mental Health (CMH) Recovery Programs provide a range of services for people with a mental illness including: individual support; self-development activities; and home-based outreach.

  • Fixed Term Position – Contract until 30th June, 2019
  • Full or Part Time Position
  • South Eastern Suburbs Location

EACH CMH Recovery works with consumers within their own community of friends, family and neighbourhood and provides a range of services for people aged 16 years and over recovering from a mental illness including:

  • Home based outreach support
  • Structured groups and activities including art programs, and skill development opportunities.
  • Support to transition to the National Disability Insurance Scheme
  • Access to internal allied health interventions
  • Coordinated care with other key stakeholders

EACH CMH Recovery have an opportunity for a Full Time Community Support Worker located within the South East Region - Narre Warren and Cranbourne to provide a range of support to consumers, tailored to meet their individual needs and successfully transition to NDIS. The Community Support Worker will work collaboratively with colleagues, and other services that are co-located.
