Aveo, a leading retirement owner/operator is seeking an experienced Registered Nurse to join our team as a Clinical Case Manager at Aveo Freedom Aged Care Bendigo.
About The Role:
As a Clinical Case Manager you will ensure residents and their family expectations are exceeded. You will be fostering relationships and networks with community care and acute care providers as you source community services and package care to support residents in the village. You will have a genuine fondness for the elderly and will enjoy working with them and their families knowing you are making a positive difference to their lives.
What we are looking for;
- Minimum 3 year's nursing experience preferably in the aged care industry
- Current AHPRA registration Div 1
- Clinical skills and good mentoring of staff skills
- Competent with Packages and networking with providers
- Able to produce good care plans and clinical assessments
- Excellent communication skills and working collaboratively as part of a team
- Demonstrated problem solving skills
- Demonstrated ability to deliver high quality clinical care with compassion
- Intermediate level of computer competence the Microsoft Suite (Word, Outlook and Excel) and on-line network applications;
- Current Australian Federal Police check
- Current First Aid/CPR
- Current Australian drivers licence
