Gateway Health is a Not-For-Profit Company which focuses on the provision of primary health and welfare services to people at highest risk of poor health outcomes. Services are delivered by over 340 staff members at sites in Wodonga, Wangaratta and Myrtleford in Victoria, and through outreach services provided across North East Victoria and Southern NSW.
The breadth of services that Gateway heath provide is astounding. We deliver innovative programs in mental health, aged care, allied health, health promotion, counselling, disability, indigenous, refugee health and many more.
This position will provide a flexible and timely Physiotherapy service which will be focussed on support for older frail or younger disabled people (or their carers) to be more independent at home and in the community, thereby enhancing their quality of life and/or preventing admission to an acute facility or inappropriate admission to long term residential care.
This position will deliver within the Home and Community Care and Commonwealth Home Support programs. Fee for service opportunities may also exist to promote service continuity. A willingness to work seamlessly across all Gateway Health public and fee for service funded programs is required as resources permit.
This position is part of the Allied Health program and reports to the Program Manager.
