Case Manager job in Hobart for a 12 week contract
Your new company
Hays are proud to be partnering with a well-established healthcare provider in Hobart. They are committed to care for their clients particularly in the aged care sector. Due to growth in their client base they require a Case Manager to support them over the next 12 weeks.
Your new role
As a case manager you will be required to deliver:
- Effective care and support for clients to enhance capacity and resilience
- Improving functional independence
- Enhance social engagement and quality of life for the client and family carer.
You will provide cost effective and evidence based case management to reach pre-determined goals for clients to enhance their quality of life. This role is predominately aged care focused and the successful candidate will require knowledge in home care packages.
What you'll need to succeed
To be successful in this role you will have:
- Relevant tertiary qualifications
- Previous experience in a case management role
- A current Australian drivers license
