Colac Area Health currently has an opportunity for a Social Work Position to join our dynamic Allied Health Team to work as an effective interdisciplinary team member providing quality care to clients through use of competent clinical skills and interventions within the scope of Colac Area Health’s integrated services.
The successful applicants will have;
- Bachelor of Social Work degree and eligibility for AASW membership
- Previous experience in a hospital/health care setting
- Proficient clinical skills in psychosocial assessment including risk assessment, complex care planning, discharge planning, Social Work interventions and communication
- Knowledge and understanding of the Social Work role in acute care, rehabilitation, palliative care, aged care, community health and outreach services
- Experience in the provision of interventions in Loss and Grief Counselling
- Ability to work both autonomously and within multidisciplinary teams
- Demonstrated understanding of theoretical frameworks that inform Social Work practice.
- Demonstrated awareness and understanding of community resources and systems.
- Demonstrated experience in social work student supervision
- Excellent Communication and IT skills in word processing and presentations
