Where you will be working
 
Leeton Health Service is a 71 bed D1a peer grouped Community Hospital with surgery.  Of the 71 beds, 38 are residential aged care beds, four are day surgery beds and four are bassinets.  The facility provides emergency, medical, palliative, surgical, obstetric, radiology and aged care services.  A range of outpatient and community health services are also provided on the hospital grounds.

What you will be doing
 
To provide comprehensive, coordinated and integrated community nursing services to individuals and their families, who have chronic conditions, or who have recently been discharged from hospital and/or who have continuing support needs and are within the designated geographical area of the network. Community nursing care involves clinical care (chronic disease management including wound and catheter care), palliative care, carer support, illness prevention and health promotion activities.
 
Selection Criteria
 
  • Registered with the Nursing and Midwifery Board of Australia (NMBA), with demonstrated evidence to meet the NMBA's Registered Nurse standards for practice and meet the NMBA approved standards, codes, guidelines and frameworks commensurate with the position..
  • Demonstrates continuous quality improvement with the ability to: comprehensively conduct assessments; develop a plan for nursing practice; provide safe, appropriate and responsive quality nursing practice; and evaluates outcomes to inform nursing practice.
  • Demonstrated engagement in therapeutic and professional relationships, communication and interpersonal skills and the ability to work within a multidisciplinary team in a rural health setting.
