About the role

We are currently seeking to appoint an Endorsed Enrolled Nurse who lives in the lower Illawarra region to join our large national team of Community Support Coordinators working in the community. The position is a casual role of 20 hours per week with the flexibility to increase hours if required.
The successful applicant will be responsible for conducting home assessments, developing and evaluating support plans to meet the needs of the individual (Participant). They will also liaise with other health professionals and multi-disciplinary teams in order to meet service delivery goals.
There is a requirement for the Community Support Coordinator to work closely with Support Workers for each Program to ensure that they have the required skill levels to support the Participants.
Critical to the success of this role is the ability to communicate effectively (verbal & written) with a diverse group of people with the focus on service.

Skills and experience
- Endorsed Enrolled Nurse (EEN) – with current AHPRA registration
- Ability to work independently with effective communication, initiative, problem solving and time management skills
- Valid NSW Driver's License and reliable vehicle
- Australian Resident
- Electronic records/Computer Skills to support administrative processing requirements
