Using the Primary Mental Health Stepped Care program, Health Service Navigators will provide access to effective, short term mental health services, at low or no cost, for people who may not otherwise be able to access these services.
Through the stepped care approach, an individual is matched to the intervention level that most suits their needs and can enter at a level that meets to their requirements.
These are full time positions, ideally situated in the locations indicated. Travel throughout the respective region will be occasionally required.

- Toowoomba, Dalby and the Western Downs
- Warwick - Southern Downs
- Goondiwindi - Goondiwindi region
You will engage with all aspects of the health system, conduct consumer intake assessments, and assist those identified to access referral and other services, and coordinate their care and follow up.
To make an impact, you will have a strong knowledge, experience and qualifications relating to the mental health system, and a capacity to engage with a range of service providers and consumers.
You will be supported by the award-winning regional experience of Rhealth and the Darling Downs and West Moreton Primary Health Network's Health Service Navigators.
