Your role:

Work within a multidisciplinary team to provide mental health case-management services to adults with a chronic and/or severe mental illness and adjunct co-morbidities. Case-management will include assessment, intervention and treatment as well as group work as required, in accordance with professional standards and legislation (Mental Health ACT 2016; State and national plans and policies).

Your team:

The Mental Health and Alcohol and Other Drugs Division is focussed on the delivery of holistic, recovery orientated evidence based interventions enabling consumers to build strength and resilience whilst continuing to live at home with meaningful support systems. The Continuing Care Team works individually and in group settings to provide ongoing mental health interventions.

Conditions of employment:
