Location: Wallsend Campus
This role is with the Greater Newcastle Dementia Advisory Services within Aged Care Services. The position requires a self-motivated Registered Nurse who will facilitate people living with dementia and their families to live at home and provide support during periods of changing care needs. This position will be responsible for working collaboratively with Aged Care Services the Community Dementia Program, Geriatricians, GPs and support services in the local region. The clinician will provide episodic case management, advocating and guidance during periods of transitioning care, facilitate groups and provide education for people with dementia and their support network and staff.
An eligibility list will be created for future similar vacancies, both permanent and temporary.
