About the Role
The Hospital in the Home Clinical Nurse is accountable at an advanced level for the coordination of clinical practice delivered in a clinical speciality. This position provides leadership and support to nursing and medical staff and supports the delivery of competent, best practice quality nursing care, while working in collaboration with the line manager of the unit. The Clinical Nurse will provide a high level of clinical care to Hospital in the Home patients, work within a team environment, and have excellent time management, interpersonal skills and work well within the multidisciplinary team.
The Hospital in the Home service is a 7 day a week service (including public holidays) (day shift only). Hospital in the Home (HITH) provides care in the community setting for acute conditions requiring clinical governance and monitoring that would otherwise require treatment in a traditional inpatient hospital bed. HITH nurses also work closely with Post Acute Nurses in wound clinic.
Hospital in the Home (HITH) provides care in the community setting for acute conditions requiring clinical governance and monitoring that would otherwise require treatment in a traditional inpatient hospital bed. Care provided by the HITH service is intended to substitute a full hospital admission or a component of a hospital admission. Patients must require daily treatment and or monitoring to qualify for entry into HITH. Patients are transferred to HITH “virtual ward” as a continuous episode of care. Patients are admitted to HITH under the responsibility of an authorised medical officer who is able to ensure a medical management plan is established and coordination of care is achieved.
Salary Information:
Total remuneration value up to $90,325 p.a. comprising salary between $44.28 – $47.41p.h.; Nurse Grade 6; employer contribution to superannuation (up to 12.75%); annual leave loading (17.5%); Temporary Part Time (64 hrs p.f); Applications will remain current for the duration of the vacancy.
