The Role

An opportunity has become available for a Clinical Nurse Consultant (Advanced Care Planning) to join Gold Coast Health (GCH) where the focus of our 9000-strong team is on providing world-class health care to over 550,000 people living in the Gold Coast, and northern New South Wales region.

You will take responsibility for

  • Strategically developing and supporting a framework for Advanced Care Planning (ACP) for the Hospital and Health Service that achieves the Key Performance Indicator (KPI) requirements as outlined in the Department of Health Service Level Agreement.
  • Training and supporting staff to improve and sustain uptake of ACP with patients within their clinical area.
  • Integrating and operationalising key objectives and strategies that promotes and supports education, learning, research and workforce development within ACP context.
  • Developing specialised education resources for patient/carer community to be utilized by other health care professionals.
  • Foster and encourage active patient engagement in the development of health care goals which promote self-management and seek to improve patient health literacy.

Requirements for the role

  • A Bachelor of Nursing, Midwifery or equivalent is a mandatory qualification for this position.
  • Appointment to this position requires proof of qualification and registration with the Australian Health Practitioner Regulation Agency (AHPRA). 
  • It is a condition of employment in this role for the employee to be, and remain, vaccinated against the following vaccine preventable diseases during their employment: measles, mumps, rubella, varicella (chicken pox), pertussis (whooping cough) and hepatitis B.
  • Experience in senior nursing positions within a relevant specialty is highly desirable.

 Salary information

  • Permanent full time
  • Base salary ranging from $107 804 - $117 037 per annum
  • Up to 12.75% employer superannuation contribution
  • 17.5% leave loading

About the Unit

Advance Care Planning (ACP) is a system-wide approach utilised to effectively identify those patients who will most likely benefit from ACP conversations. The Gold Coast Hospital and Health Service (GCHHS) ACP patient-centred model ensures there are ongoing person-centred ACP discussions between the patient within the Hospital System that are linked to General Practice Services and Residential Aged Care Facilities.
