The vision for South Eastern Sydney Local Health District (SESLHD) is ‘exceptional people, healthier lives’. SESLHD is committed to enabling our community to be healthy and well, and to providing the best possible compassionate care when people need it.

The Aged Care Assessment Program (ACAP) is a Commonwealth funded service which provides older Australians with access to a nationally consistent comprehensive aged care assessment. The SESLHD ACAP service operates as a single entity from four geographical sites across the District including Randwick, Waverley, Kogarah and Sutherland. The SESLHD ACAP Intake service provides a critical function in enabling access for SESLHD clients and their carers to the ACAP service and acts as the primary interface between SESLHD ACAP and the My Aged Care system. The service is responsible for processing, triaging and scheduling a high volume of ACAP referrals received electronically via My Aged Care on behalf of the district.
The SESLHD ACAP Intake Manager will perform a District wide role in leading and managing the operations and staff of the SESLHD ACAP Intake Service to facilitate delivery of a quality, timely and cost effective service in order to achieve ACAP business and service objectives and to meet ACAP Commonwealth Key Performance Indicators.
