Western Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care for people living in the Auburn, Blacktown, and The Hills Shire, Holroyd and Parramatta Local Government Areas (LGAs) and tertiary care to residents of the Greater Western Region. What you'll be doing

This position is responsible for coordination, change management, service development and reporting of the new service delivery model for the new Community based Palliative Care service provided by Silver Chain in WSLHD. In particular the position will oversee and coordinate project responsibilities and contract requirements between WSLHD and Ministry of Health (MoH) as outlined within the service agreement with MoH and assist the Executive Director Integrated and Community Health on operational matters as they arise.
 
The Program Manager (SII) supports  the overall program management for the WSLHD of the SII for Silver Chain community based palliative care services.

Qualification: Tertiary qualifications in an appropriate discipline and/or demonstrated equivalent industry experience as deemed appropriate by the employer

  1. Demonstrated extensive project management experience.
  2. Tertiary qualifications in an appropriate discipline and/or demonstrated equivalent industry experience as deemed appropriate by the employer
  3. Demonstrated ability and willingness to engage and consult with relevant multidisciplinary stakeholders internal and external to the organisation.
  4. A proven ability to lead and influence teams or networks to develop and achieve common goals which lead to improved patient health outcomes and experience.
  5. High level verbal and written communications, interpersonal, facilitation and negotiation skills.
  6. Demonstrated ability to plan, implement, evaluate and disseminate project outcomes.
  7. Demonstrated ability to meet competing work demands within a large complex environment, and apply sound judgement to problem solving.
